I certify that the statements made in this application are true and correct and have been given voluntarily. I understand that the Animal Rez-Q, Inc will require me to have a valid driver’s license and proof of auto insurance. I understand that the information provided on this application may be disclosed to any party with legal and proper interest, and I release the Animal Rez-Q, Inc from any liability while a volunteer of the corporation.

A volunteer is someone who chooses to act in recognition of a need, with an attitude of social

responsibility and without concern for monetary profit. Volunteers enhance and represent the work of the Animal Rez-Q, Inc, 501(c)3 intertribal nonprofit. A volunteer is not an employee and is not compensated for his or her service.
